If you’ve been scrolling through social media or researching breast augmentation lately, you’ve likely seen the name Motiva. They were FDA approved in September 2024 and they have made a big splash on social media with endorsements from celebrities like Meghan Trainor. These implants are currently the “newest” silicone options on the U.S. market.
As a plastic surgeon, I get questions about Motiva almost daily. Are they worth the hype? How do they differ from the brands we’ve used for decades? Here is what you need to know.
1. A New Era of Texture
For years, the U.S. market shifted away from “macro-textured” (rough) implants due to their association with BIA-ALCL (a rare type of lymphoma). However, texture does have a benefit: it helps the implant “grip” the tissue, which can lead to lower rates of capsular contracture (the tightening of scar tissue around an implant).
Motiva’s SmoothSilk®/SilkSurface® is a “nanotextured” shell. It is classified as smooth by the ISO, but it has a microscopic surface that the body’s cells interact with more favorably than traditional smooth shells.
- The Safety Data: In clinical studies, Motiva reported a very low rate of capsular contracture rate of around 0.5%.
- The ALCL Question: To date (March 2026), there have been no confirmed cases of BIA-ALCL associated with Motiva’s nanotextured surface.
2. A More Natural Profile
Traditional implants come in two main shapes: round or “teardrop” (shaped). Shaped implants look more natural but they are typically macro textured and quite firm to hold them in place, and even so, have the risk of flipping in the pocket.
The Motiva Ergonomix® is a game-changer because it is a round implant that acts like a shaped one.
- ProgressiveGel Ultima®: This is a highly elastic, soft cohesive gel.
- Gravity-Responsive: When you are standing, the gel shifts to the bottom of the shell to create a natural teardrop slope. When you lie down, it spreads out like natural breast tissue.
3. Round Implants: For Maximum Lift
Of course, not everyone wants a “natural” slope. Many of my patients seeking a Mommy Makeover or a more athletic look prefer a fuller “upper pole” (the top part of the breast).
- Motiva Round Implants: These use a more “form-stable” gel that stays rounder and provides more projection. If you want that visible “pop” in a bikini or a push-up bra, the round option is usually the better choice.
4. Safety: New to the US, but Not to the World
While Motiva has only been in the U.S. for about 18 months, they aren’t “experimental.” They have been used in Europe and South America since 2011.
- US Clinical Trials: They have been under intense study in the U.S. since 2019.
- Low Rupture Rates: Long-term data shows a consistently low rupture rate, giving both surgeons and patients peace of mind.
5. Are They Right for You?
Motiva Ergonomix is an excellent choice for a patient looking for a “subtle” augmentation, particularly if we are considering placing the implant above the muscle. Because the shell is so soft and the gel mimics natural tissue, the risk of “rippling” (visible waves under the skin) can be lower in some patients.
However, Motiva is just one tool in our kit. All FDA-approved brands—including Mentor, Sientra and Allergan—are safe and reliable. The “right” implant depends on your:
- Existing breast tissue and skin elasticity.
- Desired size and profile.
- Preference for placement (Above vs. Below the muscle).
