Lift or Lift with Implant? Choosing the Right Solution for Volume Loss After Pregnancy

Many patients visit me seeking to restore their breast appearance after the changes that come with pregnancy and breastfeeding. Often, they know they want a “refresh,” but they aren’t quite sure which surgical path will get them there.

During a consultation, my job is to act as a guide to help you uncover your true goals. The very first question I ask is: “Do you want your breast size to be larger than it is right now?”

Your answer to that single question almost always determines the direction of our surgical plan.

Option 1: The Breast Lift (Mastopexy)

If you are happy with your current breast volume but frustrated that everything has shifted downward, a lift alone is often the best solution.

  • The “Push-Up” Test: A great way to visualize this at home is to lift your breasts with your hands to where you want them to sit. If you like what you see and don’t feel the need for more “bulk,” you are likely a candidate for a lift.
  • What a Lift Does: It removes excess skin and tightens the internal tissue to reposition the breast higher on the chest wall.
  • The Trade-Off: A lift improves shape and “perkiness,” but it will not create a round, full upper pole (the top of the breast) in the same way an implant does. While we can sometimes use fat grafting to add subtle fullness to the cleavage area, the result remains more natural and teardrop-shaped.

Option 2: The Lift with Implant (Augmentation-Mastopexy)

If you want to restore lost volume or increase your overall cup size, we look toward implants.

  • The Benefit: Implants provide that “round” look in the upper pole that many women desire. They provide a predictable, consistent volume that a lift alone cannot achieve.
  • The Commitment: I am very transparent with my patients that choosing an implant is a long-term commitment. Implants are not lifetime devices; they require regular monitoring and will eventually need to be replaced. They are best suited for patients who truly want that increase in size and are comfortable with the maintenance involved.

The "Tissue Trade-Off": A Note on Safety

Sometimes, a patient tells me, “I want the roundness of an implant, but I don’t want to be any bigger than I am now.”

To achieve this, a surgeon would have to remove healthy, natural breast tissue to “make room” for the implant volume. As a general rule, I do not recommend this. If you ever decide to have your implants removed in the future, that natural tissue is gone forever and cannot be recovered. My philosophy is to preserve your natural anatomy whenever possible, ensuring your body remains healthy and proportional for the long haul.

Which Path is Yours?

Ultimately, the choice comes down to your lifestyle and how you want to feel in your clothing. If you want a more youthful position but prefer to avoid the maintenance of an implant, a lift is a beautiful, low-maintenance option. If you’ve always wanted more volume and are ready for the “round upper pole” look, an augmentation-mastopexy may be the answer.
